Aplication of Pamidronate (Bifosfonate) in patients suffering from Osteogenesis Imperfecta

Osteogenesis Imperfecta (O.I) groups diverse symptomology of a hereditary disease in which there is a collagen disorder, that is a present protein in connective tissue, especially in the bone.

As a consequence, there is bone fragility, frequent fractures caused by minimum trauma or even espontaneously, bone deformities and pain, and low height in most cases.

Most patients have blue sclera (the eye white portion), small, stained teeth, lowering of audition and muscle strength.

However, there is no a “healing” treatment for the disease until future genetics can achieve it; nowadays, it is posible to treat it with special medication to imporve the quality of the bone in order to avoid frequent fractures and another signs of the disease. When increasing the bone density, the patient is able to bear his own weight and able to practice sports activities, improving his quality of life.

At the Institute of Orthopaedics and Plastic Surgery, the treatment of O.I. is done by using Bifosfonates. This medication accumulates into the bone and make it more resistant..

Most used bifosfonate for treating O.I. is Pamidronate, intravenous injection and can be used in children and adults, but authors like Glorieux (Canada) has demonstrated that is more convenient to start as soon as possible, even in children less than 2 years old.

Starting with a 0.5 mgr/kg dosis of pamidronate dosis, an slow I.V. is injected for 3-4 hours during 3 days. Medical supervision during application. Cycles must be repeated every three months, at least for 1 or 2 years.

Pamidronate side effects are few: Mild fever, redness, swelling or pain at injection site, stomach pain, loss of appetite, nausea, constipation or initial increase in bone pain may occur. These effects are not present in most patients and they are controlled easily with medication.

It is recommended to take calcium (800-1000mg) and vitamin D (400 u) a day.
